How is the Patient Centered Medical Home being defined in Louisiana?

The Louisiana Health Care Quality Forum (Quality Forum) was born of stakeholder action to move forward these recommendations from the Redesign Collaborative for reform and rebuilding of the health care delivery system. The Quality Forum is a private, not-for-profit organization with a volunteer board of directors representing a cross section of public and private insurance purchasers, patient advocates, providers, physicians and insurers in the state. The Quality Forum organized itself along lines that reflected the health care reform efforts, including forming a Health Information Technology Committee, a Quality Measurement Committee, a Communication and Outreach Committee, and a Medical Home Committee. The Medical Home Committee's formed in September of 2007 with representation from the major medical home demonstration projects across the state, payers, physicians, specialists, mental health professionals, business leaders and hospitals.

The top priority for the committee was establishing a common definition and measurement criteria for the Patient-Centered Medical Home (PCMH) that could be adopted by the Quality Forum Board of Directors. In January 2008, at the recommendation of the Medical Home Committee, the Quality Forum Board endorsed the nationally recognized definition of a medical home as: "An approach to providing comprehensive primary care that facilitates partnerships between individual patients, and their personal physicians, and when appropriate the patient's family." 

An ultimate goal of the Quality Forum is achieving universal access to quality primary care, currently best defined as the PCMH, for all citizens of Louisiana, irrespective of source of health insurance, geography, age or burden of illness. Though much language contained in the definition of the PCMH focuses on the "physician", the Quality Forum believes that any design of a PCMH must be responsive to the locale of the individuals accessing care and the available resources. The definition of the model does not require a particular degree or license; however, it does require that functions and outcomes are delivered in a measurable manner by licensed providers. At times, therefore, it is not required that the PCMH be physician directed.
